In November 2008, UHN participated in a one-week Accreditation by Accreditation Canada .
Accreditation is a peer review process by Accreditation Canada that measures a hospital’s programs, services, policies and procedures according to national standards of excellence.
Accreditation is a process of continuous quality improvement. All acute care organizations participate to evaluate and improve quality and safety of services. The process is mandatory for teaching hospitals like UHN, to maintain our status as an organization committed to teaching, research and continuous learning.
UHN completed our Accreditation with fantastic results, receiving a perfect score on 1,509 out of 1,521 standards.
Accreditation – Main Components
- Self-assessment by UHN staff: The Accreditation team rolled out a Patient Safety Culture survey at the end of February 2008, and Team Questionnaires in April 2008. The results generated quality improvement roadmaps, which are stoplight reports of the responses and priority ratings. UHN also submits indicator data to Accreditation Canada on an annual basis. The self-assessment helps us identify gaps and develop action plans.
- On-site survey visit by Accreditation Canada surveyors: November 17-21, 2008: Six surveyors visited UHN and assessed priority processes using an interactive approach that enabled them to follow or ‘trace’ a client and/or process throughout the organization. Tracers may involve anything from emergency preparedness, communication, episode of care or service, medication management, medical devices and equipment use, and infection control, among others.
Accreditation Canada
In the late 1950s, health organizations across the country identified a need for national standards and a program to recognize and accredit health care facilities that meet those standards.
As a result, The Canadian Council on Health Services Accreditation was established in 1958 and has become the major national accrediting body for health services in Canada. Participating in accreditation is voluntary and, to date, approximately 1,800 member organizations are accredited by Accreditation Canada. The Canadian Council on Health Services Accreditation has since been renamed Accreditation Canada – Driving Quality Health Services.
