Your first contact with our team will be in the multidisciplinary clinic at PMH and TGH.  Your nurse at the PMH site and the Clinical Nurse Coordinator (CNC) at the TGH will coordinate appropriate services to ensure a seamless and timely transition within the episode of care.

A tour of the surgical in-patient unit at TGH is provided by the CNC if requested to familiarize patients and their family of the unit, roles and responsibilities of specific team members.
