Referral Information for Physicians

When to refer a patient to our clinic:
A neuromuscular consultation is useful for patients with the following:

  • muscle disease (e.g., myopathy, muscle weakness)
  • disorders of the neuromuscular junction (e.g., myasthenia gravis)
  • disorders of the peripheral nerves (e.g., carpal tunnel syndrome, polyneuropathy)
  • anterior horn cell diseases

Referrals for EMG tests:

  • single-fibre EMG - useful for diagnosing myasthenia
  • EMG recordings - can be helpful in some patients with movement disorders (Note: these referrals should be made by a movement disorder specialist)

When NOT to refer a patient:
A neuromuscular consultation is generally unhelpful in patients with no clinical findings who complain of chronic pain or fatigue, or in patients with symptoms without clinical findings where compensation is involved.

How to book an appointment:
Before booking an appointment, please fax the following information to (416) 603-7096:

  1. Referring doctor's name and fax number
  2. Patient's UHN MR number (from the patient's blue hospital card) if the patient has been seen at UHN before
  3. Patient's day and nighttime phone numbers
  4. Diagnosis
  5. Reason for referral request
  6. Recent consultation note, giving details of the history, findings, and investigations to date

We will contact the patient directly using the telephone numbers you provide. The report will be faxed to you about 2 days after the test is completed.
